Venous subcutaneous reservoir as a vascular access in the oncological patient [Reservorio subcutaneo venoso como acceso vascular en el paciente oncologico]

Resumen:
We present 4 years of experience with the management of 135 oncologic patients that required the placement of a Port-A-Cath catheter. The most frequent associated malignancies were: Breast Ca. 34.8%, Bronchopulmonar Ca. 14% and lymphoma 9.62%. 97.7% of the catheters were placed in the superior vena cava, and in 2.2% of the patients the catheter was placed in the inferior vena cava. There was no operative mortality. Operative morbidity in 2.2% of the patients: 2 had malpositioning of the catheter into the internal jugular vein and 1 had infection of the surgical wound. A complete follow-up was achieved in 80% of the cases from 0.3 to 47 months, during which 50% of the patients died at 8.14 months average, due to the progression of their oncologic disease. Non-operative morbidity in 4.62% of the patients: 1 with skin necrosis due to subcutaneous infusion of Adriamycin, 1 partial fracture of the catheter, 1 with axillo-subclavian vein thrombosis, 1 with thrombosis of the reservoir and one with spontaneous migration of the catheter. In the rest of the patients (95.37%), their catheters functioned without problems and were used every 28 days on the average. As seen in the present study, we can conclude that in the oncologic patient, the use of the Port-A-Cath catheter as a subcutaneous venous reservoir for central vascular access has no operative mortality, minimal morbidity and was of great help and comfort in the chemotherapeutic management of these patients.
