Valor del VPH como indicador de pronóstico en pacientes con carcinoma epidermoide de cabeza y cuello

Background: The human papilloma virus type 16 (HPV-16) has been identified as a prognostic factor in patients with squamous cell carcinoma of the head and neck (SCCHN), the virus has been acknowledged as a prognostic factor. The goal of this paper is to know whether HPV is of prognosis value in patients with SCCHN. Methods: Retrospective evaluation of patients with SCCHN; we analyzed survival, tumor location, stage, general condition (ECOG), kind of treatment and HPV association identified by PCR in tumor tissue. Descriptive analysis for survival with Kaplan-Meier and Cox regresion, with 95% confidence interval. Results: 179 patients were included, 119 (66.5%) men; 64 year-old average age. 34% located in the oral cavity and 33% in the larynx. Factors associated with survival: ECOG (RR = 11.3; 2.6-48), clinical stage (RR = 7.8; 1.7-34), age over 70 years old (RR = 3.5, 1.4-8.5) and non-surgical treatment (RR = 2.3; 1.4-3.8). The overlife media for patients with HPV16 was 22 months vs. 28 months in those with other kinds of virals. HPV infection was not associated to the prognosis in this series.
