Morphological observations in 23 cases of gastric adenocarcinoma [OBSERVACIONES MORFOLOGICAS EN 23 CASOS DE ADENOCARCINOMA GASTRICO]

In a cross-sectional prolective study that was recently carried out in the National Cancer Institute at Mexico City, in order to evaluate the utility of the total serial-step section of the stomach in gastric cancer, intestinal-type adenocarcinoma was more frequently seen. Of 23 eligible patients, 12 (52.2%) had intestinal-type adenocarcinoma (IA) and 11 (47.8%) diffuse-type adenocarcinoma (DA), in a 1.1 to 1 proportion. Since this was a unique study because of the accesibility to complete histological analysis of the urgical specimens, it was feasible to determine accompanying lesions and to be emphatic in those related with gastric carcinogenesis. Thus, in the IA group there were intestinal metaplasia with or without dysplasia in 10/12 cases (83.3%), follicular gastritis in 11/12 cases (91.7%) and Helicobacter pylori microorganisms in 8/12 cases (66.7%). On the other hand, the DA group showed intestinal metaplasia with or without dysplasia in 8/11 (72.7%) though in less extent that in the former, follicular gastritis in 6/11 (54.5%) and Helicobacter pylori microorganism in 4/11 (36.4%). These observations are comparable with those reported in high-risk areas for gastric carcinoma, notwithstanding the size of the sample. We considered that gastric carcinoma in Mexico is a changing entity, and in order to estimate its general behavior, it is necessary to conduct multicentric studies that allow us to know in detail, its regional histopathologic characteristics.
